The dislocation usually occurs forward and downward, causing the labrum (a ring of connective tissue that surrounds and enlarges the glenoid cavity) to tear away from the glenoid cavity (= Bankart lesion). Once the labrum has torn, it will not heal back into its original position on its own. <\/p><p>After repeated dislocations, the labrum degenerates completely until it is completely used up. After such an injury, it is also necessary to determine whether there is instability in only one direction or in several directions (unidirectional vs. multidirectional instability).<br><strong>X-rays<\/strong> are essential after a fresh trauma in order to determine the position of the humeral head in relation to the glenoid cavity (joint concavity) and to detect any accompanying bony injuries.<br><strong>Magnetic resonance imaging (MRI<\/strong> ) should always be performed in order to detect the labral tear (Bankart lesion) that almost always occurs and to identify concomitant injuries. Physiotherapy is therefore usually the method of choice for older patients with lower demands on the shoulder. <\/p><p class=\"\">The <strong>treatment plan is individualized<\/strong> depending on the age of the patient, the accompanying injuries and the requirements of the injured shoulder.<\/p><p class=\"\">The most common procedure is <strong>labral refixation<\/strong>, in which the injured labrum is reattached to the joint socket and the joint capsule is slightly tightened as part of a shoulder arthroscopy (joint endoscopy). This is a relatively minor procedure that is sufficient in many cases. <\/p><p class=\"\">However, a precise examination and measurement of the anatomy is required before surgery.
