data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p data-pm-slice=\"1 1 []\"><span data-color=\"var(--purple-10)\">The <\/span><strong><span data-color=\"var(--purple-10)\">rotator cuff<\/span><\/strong><span data-color=\"var(--purple-10)\">  of the shoulder consists of a group of four muscles, all of which originate from the shoulder blade: the subscapularis, supraspinatus, infraspinatus and teres minor muscles. The muscle group surrounds the humeral head or shoulder joint like a cuff and centers it in the shoulder joint.   <\/span><strong><span data-color=\"var(--purple-10)\">Shoulder socket<\/span><\/strong><span data-color=\"var(--purple-10)\"> (glenoid). <\/span><span data-color=\"var(--purple-10)\">Together with the ligaments of the shoulder joint, it ensures the necessary stability of the shoulder.<\/span><\/p><p><span data-color=\"var(--purple-10)\">In addition to the stabilizing effect of the shoulder joint, these four muscles are mainly responsible for the well-functioning rotation of the humeral head in the shoulder socket &#8211; they are therefore responsible for ensuring that the upper arm moves in relation to the shoulder blade. According to their function, they are referred to as &#8221; <\/span><strong><span data-color=\"var(--purple-10)\">rotator cuff<\/span><\/strong><span data-color=\"var(--purple-10)\">&#8221; is the term used. <\/span><span data-color=\"var(--purple-10)\">A problem arises when there is a tear in the rotator cuff (<\/span><strong><span data-color=\"var(--purple-10)\">rotator cuff lesion<\/span><\/strong><span data-color=\"var(--purple-10)\">) as a result of an acute injury (e.g. shoulder dislocation)<\/span>.  <span data-color=\"var(--purple-10)\">Much more frequently, however, chronic wear and tear or irritation (e.g. impingement syndrome) leads to degeneration or tear formation. This often progresses gradually and imperceptibly until symptoms suddenly appear, often as a result of minor trauma. Traumatic tears are much rarer and usually occur as a result of an <b>accident<\/b>, for example if you fall on your arm when it is bent or splayed. <\/span><\/p><p><span data-color=\"var(--purple-10)\">The most common cause of this type of rupture in the shoulder is probably <b>acute-chronic damage<\/b>. This occurs when a <b>small force<\/b> or <b>jerky movement<\/b> causes the already damaged tendon(s) to tear away from the bone. <\/span><\/p><p><span data-color=\"var(--purple-10)\">Like all structures in the body, the rotator cuff is also subject to a certain <b>ageing process<\/b> and the tendon plate decreases in thickness over the years. This can subsequently lead to a rupture. Various factors can accelerate this process, such as <b>increased strain<\/b>, repeated <b>injuries<\/b> or <b>smoking<\/b>. So-called <a href=\"https:\/\/www.sportortho-wien.at\/en\/calcified-shoulder\/\">calcific shoulder<\/a> and subacromial impingement (a phenomenon where the tendon is trapped under the acromion) are also defined as risk factors.   <\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"has_eae_slider elementor-section elementor-top-section elementor-element elementor-element-e50df1a animated-slow elementor-section-boxed elementor-section-height-default elementor-section-height-default elementor-invisible\" data-eae-slider=\"49454\" data-id=\"e50df1a\" data-element_type=\"section\" data-e-type=\"section\" data-settings=\"{&quot;animation&quot;:&quot;fadeInUp&quot;}\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"has_eae_slider elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-f13d36d\" data-eae-slider=\"97096\" data-id=\"f13d36d\" data-element_type=\"column\" data-e-type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-756a752 elementor-widget elementor-widget-heading\" data-id=\"756a752\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Diagnosis of a rotator cuff injury<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e4d81ee elementor-widget elementor-widget-text-editor\" data-id=\"e4d81ee\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p class=\"\"><strong>Magnetic resonance imaging<\/strong> is the safest method for detecting a tear, but also partial tears of the rotator cuff. Ultrasound examinations are also important, as they allow a dynamic examination (visualization in motion). X-rays provide a good bony overview of the shoulder and can show osteoarthritis in the AC joint (acromioclavicular joint), the shoulder joint and also calcific foci very well.  <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-124a5b0 elementor-widget elementor-widget-heading\" data-id=\"124a5b0\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Treatment of a rotator cuff injury<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-63ee7f2 elementor-widget elementor-widget-text-editor\" data-id=\"63ee7f2\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p data-pm-slice=\"1 1 []\"><span data-color=\"var(--purple-10)\">Younger patients or those with functional limitations usually require shoulder surgery.  <\/span>Partial ruptures, but especially complete ruptures (transmural ruptures) usually do not close during the course of the injury, as the traction of the affected muscle leads to further separation (dehiscence) of the tendon. Over the course of 5 years, the tears usually become larger and should therefore be <strong>surgically sutured (refixed), especially in younger people<\/strong>. In older people and if the injury has been present for some time, the muscle belly can become fatty (arthrophy) due to a lack of activity. Despite a successful rotator cuff suture (refixation), the desired result may not be achieved and a loss of strength may persist. Even if a complete reconstruction no longer appears possible, a partial tendon suture can usually achieve a significant improvement in mobility and discomfort.   <\/p><p>In older patients with a non-dominant hand and no overhead work, a three-month conservative treatment trial with physiotherapy can be attempted. The intact tendons can compensate for the sometimes minimal loss of function. In this way, good mobility and freedom from pain can be achieved. If pain persists and movement is restricted, surgery should not be delayed for too long.   <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"has_eae_slider elementor-section elementor-top-section elementor-element elementor-element-ce0d9a1 animated-slow elementor-section-boxed elementor-section-height-default elementor-section-height-default elementor-invisible\" data-eae-slider=\"66967\" data-id=\"ce0d9a1\" data-element_type=\"section\" data-e-type=\"section\" data-settings=\"{&quot;animation&quot;:&quot;fadeInUp&quot;}\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"has_eae_slider elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-e068efe\" data-eae-slider=\"53558\" data-id=\"e068efe\" data-element_type=\"column\" data-e-type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-def7535 elementor-widget elementor-widget-heading\" data-id=\"def7535\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Procedure for rotator cuff surgery<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-1ee9a54 elementor-widget elementor-widget-text-editor\" data-id=\"1ee9a54\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>I perform this <a href=\"https:\/\/www.sportortho-wien.at\/en\/the-operation\/\">operation<\/a> <strong>arthroscopically (keyhole technique) <\/strong>in almost all cases, so that only a few 1 cm scars remain. A so-called mini open procedure (additional 5 cm incision) is only required in exceptional cases. The patient is placed in a semi-sitting position on the operating table (beach chair position) and a standardized arthroscopy of the shoulder joint is performed. The entire joint is examined in order to view all parts of the rotator cuff. Here I place particular emphasis on the assessment of the subscapularis tendon (= anterior part of the rotator cuff), as tears in this tendon are very frequently overlooked.      <br>The bursa subacromialis is then removed below the acromion and a downward protruding bone spur (subacromial impingement) is removed. Several suture anchors are now inserted into the humerus and the sutures are passed through the torn tendon.
